Q: When will Terminations for Medical, Dental, Vision be process when a qualifying event takes place?

A: All changes will be effective immediately following a qualifying event.

 

Q: When I sign up for a Voluntary Benefit or Terminate an existing Voluntary Benefits, when will it be effective?

A: All voluntary benefits will be processing the first of the next month following (Example: May cancellation, effective on June 1st)

 

Q: How do I verify my eligibility for Medical Benefits?

A: Please reach out to Total Plan Concepts at (877) 435-2063 to verify eligibility for all Medical, Dental and Vision Benefits. (Please be advised that if you contact Cigna, Magnacare or any other carrier directly they will not have you in their system)

 

Q: If I have questions regarding my Colonial Life Voluntary Benefits or Mass Mutual Life Insurance, who should I contact?

A: Please reach out to Paramount Enrollments at (888) 613-0900 for assistance with all Colonial Life Benefits and Mass Mutual Benefits.

 

Q: When enrolled in medical benefits, when can I expect to receive my cards?

A: Your card will be mailed out 48hrs after employee is enrolled into the system. – which will be mailed to the address on file.

 

Q: If I’m enroll into dental benefits, when do I get my card.

A: You can pick up a card at your facility HR mangers office.
